How long does the survey take?

5-10 minutes. And it’s more fun than a traditional survey.

Can I do it on my phone?

Yes! You can do it on a phone, tablet, or computer.

What can I win?

After you finish, if you want to enter the draw, we’ll ask for your email address so we can contact you if you win the prize – a $100 Prezzy card.

What about privacy?

The survey results are confidential to DOC, and we only look at combined results across all participants. If you give us your email address for the draw, we keep it separate from your test results.

If you opt in to future studies about the DOC website, we’ll only use your email for that purpose. We will not share it with any third parties.

Conditions of this survey

• You’ll get one entry for the draw if you finish the survey. Doing more than one exercise won’t get you extra entries.
• The prize is a digital Prezzy card for $100.
• The winner will be randomly drawn from all entries and contacted by email 7 days after the survey closes.
• DOC staff and contractors are not eligible for the draw.
• If you are under 16, you must obtain permission from your parent/guardian before entering.
