What happens if I was issued with a DOC Lifetime Pass in 1997?
The DOC Lifetime Pass Card (1997) has been superseded by the new DOC Lifetime Pass Card (2013). Lifetime Pass cardholders who are not issued with a new card will still be able to use their Lifetime Pass Card (1997) subject to the same terms and conditions and relevant discounts applicable to the Lifetime Pass Card (2013). If you were employed by DOC as at 2 September 2013, and have a Lifetime Pass Card (1997), you will be issued with a new DOC Lifetime Pass Card (2013) and you should destroy your old card, and use your new card.
Can I get a replacement card?
The Lifetime Pass Card can be replaced when it has been lost or stolen, or if you have changed your name. Order a replacement card online. Replacement cards will incur a charge of $50 (subject to change). Payment can only be made via credit/debit card. Contact will be made with you at the phone number you provide to obtain payment. Once we have confirmed that you are eligible for a replacement card and your payment has been processed, it will take about 2-4 weeks for your replacement card to be delivered.
