Apply offline (by post) for a conforming concession
If you cannot apply for a conforming concession online then you will need to:
- Print out the application form:

- Manually fill in sections A, B, C, H and I of the application form.
- Print out the relevant pages of the Conforming Activity Schedule/s and
- In the yellow column on the right manually tick the locations you wish to apply for (do not write them into the application form (section D) by hand; simply tick them on a print out of the Schedule).
- In the green column on the right insert the number of trips/per year you wish to apply for at each location. Please note: if the maximum frequency is e.g. 15/day then you cannot apply for more than 365 trips/year.
- Mail the following documents to your nearest office - DOC Concession contacts:
- Your completed application form (with sections A,B,C,H and I completed).
- The relevant pages of the Conforming Activity Schedule/s with the locations you wish to apply for ticked.
- Payment by cheque.
The cost of your concession application will depend on the number of conforming activities (items) you wish to apply for. The standard application fee is $285+GST for up to 10 items; plus an additional $95+GST for each additional 10 items i.e:
- 11-20 items = $380+GST
- 21-30 items = $475+GST
- 31-40 items = $570+GST, etc.
You can apply for as many conforming items as you like. The more you apply for, the more it will cost you.

Note: Once your concession has been processed you will be required to provide the following documents before you can exercise your concession rights:
- A signed copy of your concession contract.
- An independent auditor's approval for your Safety Plan.
- A certificate of proof of insurance cover to the required level.
